Indications:

Tryptin is indicated for the treatment of depressive disorders, especially when sedation is beneficial. It is also used in nocturnal enuresis in children, migraine prophylaxis, tension headaches, and chronic pain management.

Description:

Tryptin is a tricyclic antidepressant with strong sedative and anticholinergic properties. It works by inhibiting the reuptake of noradrenaline and serotonin at nerve endings, thereby increasing their levels in the brain. The drug is rapidly absorbed from the gastrointestinal tract, with peak plasma levels occurring within 2 to 12 hours. It is mainly excreted through urine in the form of metabolites.

Pharmacology:

Amitriptyline acts by blocking the reuptake of neurotransmitters such as serotonin and norepinephrine at synaptic junctions in the central nervous system. Increased concentrations of these neurotransmitters help improve mood and relieve depressive symptoms. Its effectiveness in chronic pain and headache prevention may be related to both mood-enhancing and independent analgesic properties.

Dosage & Administration:

Depression: Initial dose is usually 75 mg daily in divided doses or as a single bedtime dose. Elderly patients and adolescents may require 30-75 mg daily. The dose may gradually be increased up to 150-200 mg daily if necessary. Not recommended for depression in children under 16 years.

Nocturnal Enuresis:

Children 7-10 years: 10-20 mg at bedtime.

Children 11-16 years: 25-50 mg at bedtime.

Maximum treatment duration, including gradual withdrawal, should not exceed 3 months. A complete physical examination is recommended before further treatment.

Migraine Prophylaxis: 100 mg daily.

Tension Headache: 10-25 mg three times daily.

Interaction:

Tricyclic antidepressants may enhance the sedative effects of alcohol and opioid analgesics. Concomitant use with Moxifloxacin or Terfenadine may increase the risk of ventricular arrhythmias. Disulfiram and Cimetidine may inhibit the metabolism of Amitriptyline. Use with diuretics may increase the risk of postural hypotension.

Contraindications:

Amitriptyline is contraindicated in patients with recent myocardial infarction, cardiac arrhythmias including heart block, mania, and severe liver disease. Sedation may impair the ability to drive or operate machinery, especially at the beginning of therapy.

Side Effects:

Anticholinergic effects may include dry mouth, blurred vision, urinary retention, excessive sweating, hyperpyrexia, and urinary tract dilation. Cardiovascular effects may include hypotension, postural hypotension, hypertension, tachycardia, palpitations, syncope, and rarely myocardial infarction. CNS effects may include confusion, impaired concentration, disorientation, and delusions. Allergic reactions such as rash, urticaria, and photosensitivity may occur. Other effects include bone marrow depression, nausea, vomiting, anorexia, constipation, diarrhea, weight loss, stomatitis, gynecomastia, breast enlargement, galactorrhea, and testicular swelling.

Pregnancy & Lactation:

Pregnancy Category C. Amitriptyline crosses the placenta and should be used during pregnancy only when the expected benefit outweighs potential fetal risk. It is excreted in breast milk; therefore, breastfeeding should be discontinued or the drug should be stopped depending on clinical necessity.

Precautions & Warnings:

Use cautiously in patients with epilepsy, glaucoma, urinary retention, cardiovascular disease, diabetes, thyroid disorders, hepatic impairment, increased intraocular pressure, pregnancy, and psychiatric conditions such as psychosis or mania, as symptoms may worsen.
