Esomeprazole, the S-isomer of omeprazole, is a potent proton pump inhibitor (PPI) that effectively reduces gastric acid secretion by irreversibly inhibiting the H+/K+-ATPase enzyme system (the "proton pump") located at the secretory surface of the gastric parietal cell. As a prodrug, it is absorbed in the small intestine and sequestered in the acidic environment of the parietal cell's secretory canaliculi, where it is converted into its active sulfonamide form to form a covalent disulfide bond with the proton pump. Because esomeprazole has a lower first-pass metabolism and higher systemic exposure (AUC) than its racemic predecessor, it provides more sustained and predictable control of intragastric pH. It is primarily indicated for the treatment of gastroesophageal reflux disease (GERD), the healing and maintenance of erosive esophagitis, and the eradication of Helicobacter pylori when used in combination with appropriate antibiotics. Clinically, esomeprazole is preferred for its rapid onset of action and superior acid suppression over 24 hours, which is crucial for managing Zollinger-Ellison syndrome and preventing NSAID-induced gastric ulcers. It is metabolized in the liver via the CYP2C19 and CYP3A4 pathways, which necessitates caution when co-administered with drugs like clopidogrel or diazepam due to potential metabolic interference. While generally considered safe for short-term use, chronic administration has been linked to potential risks such as hypomagnesemia, vitamin B12 deficiency, and an increased susceptibility to enteric infections like Clostridioides difficile. To ensure maximum bioavailability, it is typically administered 30 to 60 minutes before a meal, as the presence of food can significantly delay absorption, and the proton pumps are most active post-prandially.
