Levofloxacin hemihydrate is a third-generation synthetic fluoroquinolone antibiotic that exhibits potent, broad-spectrum antibacterial activity against a wide range of Gram-positive and Gram-negative pathogens, including atypical organisms like Mycoplasma pneumoniae and Chlamydia pneumoniae. Its primary mechanism of action involves the inhibition of bacterial type II topoisomerases—DNA gyrase and topoisomerase IV—which are essential for bacterial DNA replication, transcription, repair, and recombination. By disrupting these enzymes, the drug promotes irreversible DNA damage, leading to rapid bacterial cell death. Clinically, it is highly valued for its excellent tissue penetration and high bioavailability, making it a first-line or significant option for treating community-acquired pneumonia, acute sinusitis, chronic bronchitis exacerbations, complicated urinary tract infections, and skin infections. Because it is the S-enantiomer of the racemic drug ofloxacin, it possesses twice the potency of its predecessor. However, its use is accompanied by important safety considerations, including the risk of tendinitis and tendon rupture, peripheral neuropathy, and central nervous system effects. Furthermore, patients are advised to avoid co-administration with multivitamin supplements or antacids containing magnesium, aluminum, or iron, as these can significantly reduce the drug's absorption through chelation.
