Nabumetone is a non-acidic nonsteroidal anti-inflammatory drug (NSAID) belonging to the naphthylalkanone class, primarily used for the symptomatic treatment of osteoarthritis and rheumatoid arthritis. A unique pharmacological feature of nabumetone is its status as a prodrug; it is relatively inactive in its parent form but undergoes extensive hepatic metabolism to form the active metabolite, 6-methoxy-2-naphthylacetic acid (6-MNA). This active metabolite is a potent inhibitor of the cyclooxygenase (COX) enzymes, thereby reducing the synthesis of inflammatory prostaglandins. Because nabumetone is non-acidic and the active metabolite is produced only after absorption, it generally exhibits a lower incidence of direct gastric mucosal irritation compared to many other NSAIDs. It possesses a long half-life, which allows for convenient once-daily dosing, though clinicians must still monitor patients for potential systemic risks such as renal impairment, cardiovascular events, and occult gastrointestinal bleeding.
