Indications: Olmedip is indicated for the treatment of hypertension, either alone or in combination with other antihypertensive agents, to help lower blood pressure. It may also be used as initial therapy in patients who are likely to require multiple antihypertensive agents to achieve target blood pressure goals. Treatment decisions should be individualized based on baseline blood pressure, target goals, and the patient’s cardiovascular risk profile.

Pharmacology: Olmedip combines Amlodipine and Olmesartan Medoxomil. Amlodipine is a dihydropyridine calcium channel blocker that inhibits calcium ion influx into vascular smooth muscle and cardiac muscle, resulting in peripheral arterial vasodilation and reduced blood pressure. Olmesartan Medoxomil is an angiotensin II receptor blocker (ARB) that selectively blocks the binding of angiotensin II to AT1 receptors in vascular smooth muscle and adrenal glands, preventing vasoconstriction and aldosterone secretion. Together, these agents provide complementary blood pressure lowering effects.

Dosage & Administration: Patients already receiving individual components may switch to the equivalent combination dose. Initial therapy is usually started with 5 mg Amlodipine and 20 mg Olmesartan Medoxomil once daily for 1 to 2 weeks, with dose adjustment as needed up to a maximum of 10 mg/40 mg once daily. In elderly patients above 75 years, initial combination therapy is generally not recommended because the lowest available dose contains 5 mg Amlodipine, while elderly patients often require a lower starting dose of 2.5 mg Amlodipine.

Interaction: The antihypertensive effect of Olmesartan may be reduced when used with NSAIDs, including selective COX-2 inhibitors. Concurrent use with drugs affecting the renin-angiotensin system (RAS) requires careful monitoring of blood pressure, renal function, and electrolyte levels.

Contraindications: Olmedip should not be used together with Aliskiren in patients with diabetes.

Side Effects: Common side effects include peripheral edema, headache, flushing, dizziness, and fatigue. In rare cases, Olmesartan may cause severe intestinal problems known as sprue-like enteropathy.

Pregnancy & Lactation: Pregnancy Category D. Olmedip should not be used during the second and third trimesters of pregnancy because it may cause fetal injury or death. Treatment should be discontinued immediately if pregnancy occurs. It is not known whether Amlodipine or Olmesartan are excreted in human breast milk; therefore, caution is advised during breastfeeding.

Precautions & Warnings: Use cautiously in patients with volume or salt depletion due to the risk of hypotension. Care should also be taken in patients with severe aortic stenosis because excessive vasodilation may occur. In patients with severe obstructive coronary artery disease, therapy may increase the frequency or severity of angina or myocardial infarction, especially after starting treatment or increasing the dose.

Use in Special Populations: Safety and effectiveness in pediatric patients have not been established. No overall differences in safety or effectiveness have been observed between elderly and younger adults. There are limited studies in patients with renal impairment. Initial therapy is not recommended in patients with hepatic impairment.

Overdose Effects: There is limited information regarding overdose in humans. Supportive and symptomatic treatment is recommended in case of overdose.

Therapeutic Class: Combined antihypertensive preparations.
