**Liptor (Atorvastatin) Summary**


**Indications**


* **Lipid Management:** Used as an adjunct to diet to lower elevated total cholesterol, LDL cholesterol, apolipoprotein B (Apo-B), and triglycerides in patients with primary hypercholesterolemia, mixed dyslipidemia, hypertriglyceridemia, and heterozygous/homozygous familial hypercholesterolemia.

* **Cardiovascular Prevention:** Reduces the risk of cardiovascular events (e.g., heart attack, stroke) in high-risk individuals.


**Mechanism & Pharmacology**


* **Class:** HMG-CoA reductase inhibitor (Statin).

* **Action:** Inhibits HMG-CoA reductase (the rate-limiting enzyme in hepatic cholesterol synthesis), upregulating cell-surface hepatic LDL receptors to increase the uptake and catabolism of LDL.

* **Pharmacokinetics:** Rapidly absorbed (peak concentration in 1–2 hours); absolute bioavailability is ~14% (30% for total inhibitory activity). High plasma protein binding (98%). Extensively metabolized by CYP3A4 to active ortho- and parahydroxylated derivatives, which account for ~70% of systemic HMG-CoA reductase inhibition. Half-life of active inhibitory activity is 20–30 hours. Primary elimination is biliary (<2% excreted in urine).


**Dosage & Administration**


* **Administration:** Oral single dose once daily, at any time of day, with or without food. Maintain a standard cholesterol-lowering diet throughout treatment.

* **Adults (Hypercholesterolemia / CV Prevention):**

* *Starting dose:* 10 mg or 20 mg once daily (40 mg once daily if LDL-C reduction >45% is required).

* *Maintenance/Adjustment:* 10–80 mg once daily. Adjust at intervals of 2–4 weeks or more based on lipid panel response.



* **Pediatrics (10–17 years):** Starting dose 10 mg/day; maximum 20 mg/day (up to 80 mg/day for familial hypercholesterolemia under specialist guidance). Not recommended for children <10 years.

* **Renal Impairment:** No dosage adjustment necessary.


**Contraindications**


* Known hypersensitivity to Atorvastatin or any component.

* Active liver disease or unexplained, persistent elevations of serum transaminases.

* Pregnancy, planning pregnancy, or breastfeeding.


**Key Interactions**


* **Myopathy Risk:** Concurrent use with cyclosporine, fibrates, niacin, azole antifungals, or erythromycin increases the risk of myopathy/rhabdomyolysis.

* **Digoxin:** Co-administration increases steady-state plasma digoxin levels by ~20%; monitor closely.

* **Oral Contraceptives:** Increases AUC values of norethindrone and ethinyl estradiol.

* **CYP3A4 Inhibitors (e.g., Erythromycin):** Elevates plasma Atorvastatin concentrations.

* **Antacids / Colestipol:** Decreases Atorvastatin plasma concentrations without significantly impacting its overall LDL-C lowering efficacy.


**Side Effects & Safety Precautions**


* **Common Side Effects:** Constipation, flatulence, dyspepsia, abdominal pain, headache, fatigue, back pain, myalgia, arthralgia, and rash.

* **Liver Function:** Perform liver function tests before starting treatment and periodically thereafter. Use with caution in heavy alcohol consumers or those with a history of liver disease.

* **Musculoskeletal Risks:** Monitor for unexplained muscle pain, tenderness, or weakness. Discontinue if myopathy is diagnosed or if CPK/CK levels are markedly elevated.

* **Diabetes Risk:** May cause minor increases in blood glucose levels; monitor blood sugar regularly in diabetic patients.


**Pregnancy & Lactation**


* **Contraindicated.** Fetal cholesterol synthesis is necessary for fetal development. Discontinue immediately if pregnancy occurs or is suspected. Breastfeeding is contraindicated due to potential serious adverse reactions in nursing infants.
