Product Description

Pharmacology:

Telmisartan is a non-peptide Angiotensin-II Receptor Blocker (ARB). It selectively blocks the AT₁ receptor, preventing angiotensin-II-induced vasoconstriction and aldosterone secretion. This causes relaxation of blood vessels and reduces blood pressure. Telmisartan has >3,000-fold greater affinity for AT₁ than AT₂ receptors. It does not inhibit ACE (kininase-II), so it does not affect the action of bradykinin.


Dosage & Administration:

The usual starting dose is 40 mg once daily, with a dose range of 20–80 mg/day. Most antihypertensive effects appear within 2 weeks, with maximum effect generally reached after 4 weeks. It can be taken with or without food. No initial dose adjustment is usually required in elderly or renal-impaired patients. However, initial therapy is not recommended in patients ≥75 years or with hepatic impairment. In hepatic impairment, careful monitoring and slow dose titration are required. Pediatric safety and efficacy have not been established.


Drug Interactions:

NSAIDs may increase the risk of renal impairment and reduce the antihypertensive effect. Concomitant use with aliskiren should be avoided in diabetic patients. Combined use with an ACE inhibitor and ARB should also be avoided.


Contraindication:

Contraindicated in patients with known hypersensitivity to Telmisartan or any of its components.


Side Effects:

Common adverse effects include sinusitis/sinus congestion, back pain, and diarrhea. In cardiovascular risk reduction, intermittent claudication and skin ulcer may occur.


Pregnancy & Lactation:

Fetal and neonatal exposure should be avoided. Telmisartan is particularly harmful during the 2nd and 3rd trimesters of pregnancy. During lactation, discontinuation of breastfeeding or the drug should be considered based on the importance of treatment to the mother.


Precautions:

Monitor for hypotension and carefully monitor patients with hepatic or renal impairment. Avoid concomitant use with ACE inhibitors.


Overdose:

The most likely effects are hypotension, dizziness, and tachycardia. Bradycardia, increased serum creatinine, and acute renal failure may also occur.
