Megenox (Megestrol Acetate): Megenox is a synthetic progestational and antineoplastic agent indicated for the palliative treatment of advanced breast and endometrial carcinoma (recurrent, inoperable, or metastatic) and for the treatment of anorexia, cachexia, or significant weight loss in patients with AIDS or cancer. It should not replace standard treatments such as surgery, radiotherapy, or chemotherapy. Megestrol acetate exerts its antitumor effects by reducing estrogen secretion through inhibition of pituitary gonadotropins and by directly inhibiting hormone-dependent tumor cell growth. The recommended dose is 160 mg/day for breast cancer, 40–320 mg/day in divided doses for endometrial carcinoma (continued for at least 2 months to assess efficacy), and 800 mg/day (20 mL) for the oral suspension. It is contraindicated in patients with hypersensitivity to megestrol acetate and in pregnancy. The most common adverse effect is weight gain due to increased appetite, while other side effects include nausea, vomiting, edema, breakthrough uterine bleeding, gynecomastia, hearing loss, dyspnea, heart failure, hypertension, hot flashes, mood changes, hyperglycemia, alopecia, rash, and thromboembolic events (e.g., thrombophlebitis and pulmonary embolism). It should be used with caution in patients with a history of thrombophlebitis, severe hepatic or renal impairment, and elderly patients, with regular monitoring during therapy. Megestrol is not recommended during pregnancy or breastfeeding because of potential fetal harm and adverse effects in infants. There are no well-established drug interactions, but caution is advised with agents that may cause fluid retention. In overdose, treatment is supportive, as no specific antidote is available. Megenox should be stored below 30°C, in a cool, dry place, protected from light and moisture, and kept out of the reach of children.
