Arnis, which contains Sacubitril+Valsartan, is a first-in-class therapeutic combination known as an Angiotensin Receptor-Neprilysin Inhibitor (ARNI), primarily indicated for the treatment of symptomatic chronic heart failure with reduced ejection fraction. It consists of two active components that work synergistically: Sacubitril, a prodrug that inhibits the enzyme neprilysin to increase levels of beneficial natriuretic peptides, and Valsartan, an angiotensin II receptor blocker (ARB) that prevents harmful vasoconstriction and fluid retention. By simultaneously enhancing the body’s natural protective systems (natriuresis and vasodilation) and suppressing the overactive renin-angiotensin-aldosterone system (RAAS), this combination significantly reduces the risk of cardiovascular death and hospitalization. Clinically, it is often used as a replacement for traditional ACE inhibitors or ARBs in heart failure management. Key safety considerations include monitoring for hypotension, hyperkalemia, and renal impairment; furthermore, it must never be administered within 36 hours of an ACE inhibitor due to the high risk of angioedema.
