SUBA-i contains Itraconazole, a potent, broad-spectrum triazole antifungal agent used to treat a variety of systemic and superficial fungal infections, including aspergillosis, blastomycosis, and onychomycosis. It functions by inhibiting the fungal enzyme lanosterol 14-alpha-demethylase, a cytochrome P450-dependent enzyme essential for converting lanosterol to ergosterol, a key component of the fungal cell membrane. This inhibition leads to increased membrane permeability and eventual cell death. Itraconazole is highly lipophilic and exhibits significant tissue distribution, often persisting in keratinous tissues like skin and nails even after treatment ends. Because its absorption is highly dependent on an acidic gastric environment and it is a known potent inhibitor of the CYP3A4 isoenzyme, it requires careful clinical monitoring regarding stomach acidity and potential drug-drug interactions.
