Levetiracetam is a second-generation antiepileptic drug (AED) used as both monotherapy and adjunctive treatment for focal-onset, myoclonic, and primary generalized tonic-clonic seizures. Its mechanism of action is distinct from traditional anticonvulsants; it primarily binds to the synaptic vesicle protein 2A (SV2A) in the brain, which is believed to modulate neurotransmitter release and inhibit presynaptic calcium channels, thereby stabilizing neuronal hyperexcitability. Favorably regarded for its linear pharmacokinetics, it exhibits high oral bioavailability, minimal protein binding, and is not significantly metabolized by the cytochrome P450 system, leading to a low potential for drug-drug interactions. While it is generally well-tolerated, clinical monitoring is often focused on behavioral side effects, such as irritability, agitation, or "Levetiracetam-induced mood symptoms," particularly in patients with a history of psychiatric conditions.
