The combination of frusemide (furosemide) and spironolactone is a potent diuretic therapy designed to manage edema associated with congestive heart failure, hepatic cirrhosis, and nephrotic syndrome. Frusemide is a high-ceiling loop diuretic that inhibits the Na+−K+ −2Cl− symporter in the thick ascending limb of the loop of Henle, leading to significant excretion of water and electrolytes. However, frusemide often causes substantial potassium depletion. Spironolactone, a potassium-sparing diuretic and aldosterone antagonist, complements this by acting on the distal convoluted tubules to promote sodium excretion while retaining potassium. By combining these two agents, the regimen achieves a synergistic natriuretic effect while maintaining electrolyte balance, specifically mitigating the risk of hypokalemia. Clinically, this combination is particularly effective in treating resistant edema and ascites, though patients must be monitored regularly for renal function and serum potassium levels to avoid hyperkalemia.
